Botox in İstanbul: dosing, safety and how to avoid a frozen look

Medical team, Nuance Clinic

Botulinum toxin is the most-performed aesthetic treatment in the world and the most commonly over-dosed. This guide explains how a doctor assesses muscle strength, why units are not a price list, what happens between day 3 and day 14, and how to judge a clinic's safety standards before you sit in the chair.

Quick facts

  • Dose should follow muscle strength, not a per-area price list.
  • Full effect appears at day 10–14; a two-week review matters.
  • Under-treating first and adding later is safer than over-treating.
  • Toxin does not replace filler, skin boosters or resurfacing.

What it is

  • Botulinum toxin type A temporarily blocks the nerve signal to selected muscles. Lines caused by repeated movement soften, and lines that have not yet formed are prevented.
  • It does not fill volume, tighten loose skin or treat sun damage — those need filler, energy devices or skin treatments.

Who it suits

  • Forehead lines, glabellar (frown) lines, crow's feet, bunny lines, gummy smile, chin dimpling and neck bands
  • Patients seeking prevention in their late twenties and thirties
  • Not suitable in pregnancy, breastfeeding, neuromuscular disease (e.g. myasthenia gravis) or known allergy

Products we use

  • Original, licensed botulinum toxin brands including Botox (Allergan), Dysport and Xeomin
  • Product is reconstituted fresh, in front of you where you wish, and never diluted beyond protocol

How the treatment is performed

  • Dynamic assessment: we watch you frown, raise, smile and squint before deciding on injection points.
  • Skin cleaned; injections with a very fine needle, typically 4–8 points per area.
  • The whole session takes minutes; you can return to work immediately.

Safety

  • Correct dosing and depth prevent eyelid or brow drop; this is anatomy, not luck.
  • We deliberately under-treat first-time patients and add at review, because toxin cannot be removed once injected.
  • Full medical history is taken — certain antibiotics and neuromuscular conditions matter.

Before your appointment

  • Avoid alcohol for 24 hours.
  • Tell us if you are on aminoglycoside antibiotics or have any neuromuscular disorder.
  • Do not book within two weeks of a major event, so there is time to fine-tune.

Aftercare

  • Stay upright for 4 hours; no lying face down.
  • No rubbing or facial massage for 24 hours.
  • No gym, sauna or hammam for 24 hours.
  • Active facial expressions in the first hours may help uptake.

Results and longevity

  • First changes at day 3, full effect at day 10–14.
  • Effect lasts 3–5 months; regular treatment often extends the interval over time.
  • Deep static lines etched into skin need filler, skin boosters or resurfacing in addition.

Possible side effects and risks

  • Common: small bruises, brief headache, transient heaviness.
  • Uncommon: brow or eyelid ptosis (temporary, weeks), asymmetry — usually correctable at review.
  • Rare: allergic reaction, flu-like symptoms.

Follow-up

A two-week review is part of every toxin treatment, with adjustment where needed at no extra charge.

Questions about this treatment

Will I look frozen?

Not with individualised dosing. We aim to weaken the pull of specific muscles, not to eliminate movement — most patients keep full expression with softer lines.

Is Botox safe long term?

Botulinum toxin has decades of clinical use in medicine and aesthetics. Long-term regular use typically means needing less product over time.

How old should I be to start?

When dynamic lines start to stay visible at rest — for many people that is between 27 and 35. There is no fixed age.

Can I combine it with filler?

Yes, and it is usually the better plan: toxin handles movement, filler handles volume.
